The Home Care Nurse provides medical care and support to patients in their homes. They assess patients’ conditions, develop care plans, and administer medications. The Home Care Nurse also monitors vital signs, provides wound care, and assists with activities of daily living. They educate patients and their families on healthcare management and collaborate with other healthcare professionals to ensure comprehensive care.
Responsibilities
- Assess patients’ conditions and develop individualized care plans.
- Administer medications and perform medical procedures as required.
- Monitor and record patients’ vital signs and symptoms.
- Provide wound care and assist with personal hygiene.
- Educate patients and their families on healthcare management and treatment plans.
- Collaborate with other healthcare professionals to coordinate care.
- Maintain accurate and up-to-date medical records.
- Review patients’ progress and modify care plans as necessary.
- Provide emotional support and counseling to patients and their families.
- Ensure a safe and clean environment for patients.
- Bachelor’s degree in nursing or a related field.
- Valid nursing license.
- Proven experience in home care nursing.
- Knowledge of medical procedures and protocols.
- Strong communication and interpersonal skills.
